Audit-log completeness is a v1 KPI: ≥ 99% of runs have run id, source url,
rubric id, and scores recorded. Held runs (extract disagreement > threshold, content-hash duplicate, or rate-limit) are recorded
without scores. The seed row above is the PRD’s canonical example; everything below it is a real submitted run.
